Waiver of Liability: By signing below, I acknowledge and agree that participation in basketball activities at FBC or i20 Sports Complex involves inherent risks, including the risk of injury. I, on behalf of myself and my child, hereby release, waive, and discharge FBC, i20 Sports, their directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, or causes of action for injuries, damages, or losses that may arise from participation in any activities, whether caused by negligence or otherwise.

Assumption of Risk: I understand and accept all risks associated with my child’s participation in basketball activities, including but not limited to physical injury, illness, or property damage.

Medical Authorization: In the event of an emergency, I authorize FBC and i20 Sports staff to secure medical treatment for my child as deemed necessary.

Lease Agreement: I understand that any equipment or facilities provided are to be used responsibly and returned in good condition. I agree to be responsible for any damages caused by misuse or negligence.

Acknowledgment: I have read and understand this waiver and lease agreement. I voluntarily sign this document and agree to its terms.
