Waiver and Release Form:

For the adult named above, I hereby give my full consent and approval for myself to participate as a team member in the sport. I understand that there are certain risks of injury inherent in the practice and play in this sport. I hereby certify myself that I am fully capable of participating in the designated sport. I am healthy and have no physical or mental disabilities or infirmities that would restrict participation. I understand that there are physical risks in every sport, therefore i take full responsiblity for my own injurys and expenses. I do here waive, release and hold harmless to i20 Sports and First Baptist Church, GWCC and its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by myself in the normal course of participation in the designated sport and the activities incidentals, whether they result or negligence or any other cause.
